2025 Anthem Wellness Program - October 15th Deadline Reminder

This is a reminder that all employees (and spouses, if applicable) enrolled in an SMP-sponsored medical plan must individually complete the annual Wellness Program through separate accounts by October 15th, 2025.

Failure to meet the October 15th, 2025, deadline will result in a $600 premium increase in 2026. To be fully compliant with the Wellness Program, you only need to complete one activity.

Required Activity (Claims Based):

  • Annual Preventive Exam
  • Or Well Woman Exam (if applicable)

*Claims-based means that once your provider submits the preventive exam claim using your Anthem insurance, your rewards will automatically reflect on the Anthem portal and Sydney app.

Tips to Avoid the 2026 Surcharge:

    • Complete the 2025 Wellness Program requirement.
    • Be aware that claims may take up to 60 days from the date processed by your primary care provider (PCP) to display on the portal.
    • Avoid booking last-minute appointments with your provider.
    • Confirm with your provider that you are receiving a “preventive exam” only.

Important Reminders:

  • Date of service can be between October 16th 2024 and October 15th 2025.
  • Our medical plan allows for one annual physical per year and does not require 12 months between appointments.
  • No Biometric form is required to complete the Wellness Program.
